To get your SunDial pass by mail, please print out and fill in this form. Mail the completed form along with your check or money order payable to "SunLine Transit Agency" to the address below.  Allow 7 to 10 working days to receive your pass.  Please do not send cash.  Be sure to include your name and return address.  All passengers purchasing SunDial passes must be ADA Certified.
